FDA Warning on Topamax

In March 2011, the U.S. Food & Drug Administration (FDA) issued a warning about the development of cleft lip and/or cleft palate (oral cleft defects) in infants born to mothers who took the anticonvulsant drug Topamax during pregnancy. As a result of these problems, FDA re-classified Topamax as a Pregnancy Category D medication, which means that there is direct evidence of birth defects based on human data.

“Health care professionals should carefully consider the benefits and risks of topiramate when prescribing it to women of childbearing age,” said Russell Katz, M.D., director of the Division of Neurology Products in the FDA’s Center for Drug Evaluation and Research. “Alternative medications that have a lower risk of birth defects should be considered.”

Topamax Birth Defects

Oral Cleft Birth Defects

An oral cleft birth defect occurs when a portion of the lip fails to fuse together in utero during the first trimester of pregnancy. Depending on the severity of the cleft, the defect may appear as a small notch in the edge of the lip, or it may extend into the nose or gums. Because the lips and the palate develop separately in the womb, it is possible for a baby to be born with a cleft lip only, cleft palate only, or both a cleft lip and cleft palate. With a separation or opening in the lip, eating problems can occur because food and liquid pass from the mouth back through the nose. Additionally, babies with oral cleft birth defects are at an increased risk of developing ear infections since they are more prone to fluid buildup in the middle ear. Speech and dental problems are also commonly reported in children with oral cleft defects.
